School and Sport Physicals
School and Sport Physicals are Now Available Through August 31, 2020.
Kindergarten – 7th Grade
An annual school physical is a good time to touch base with your child’s health care provider. Aside from ensuring your child is up-to-date on vaccines, your doctor will review your child’s family history and personal health, perform a basic eye exam and a basic hearing test, monitor development and growth patterns, and check their overall physical health.
*Fee is paid at the time of the visit and will not be sent to insurance. Lab and immunizations are not included. Students must provide a current immunization card. Please bring glasses or contacts to physical. If requesting to send to insurance, visit will be billed as Preventative Medicine. Please check with your insurance provider for coverage.
9th – 12th Grade
This physical is a chance for your doctor to make sure it’s safe for your child to begin a new sports season or sport. The doctor will look for any health problems that could possibly be a danger during a game or practice. He or she might find something that can be treated or managed so your child doesn’t have to sit on the sidelines.
*No insurance claims will be filed. Students must provide a completed and signed school physical form. Payment due at the time of physical. Please bring glasses or contacts to physical. Due to Scoliosis check, no sports bras please.
